eBrigade Forms module
Field checklists that come back without re-entry
You design custom forms with eight field types, link them to your activity types, and your teams fill them in directly from the event page. Responses are time-stamped, attributed to their author and exportable to Excel with a statistical summary sheet.
These buttons are about this optional module: add-on pricing and a 1-month trial of the module.

What is the eBrigade Forms module?
Custom forms linked to your activity types, filled in from the event page and exportable to Excel with a statistical summary. It is available as an option so that each organisation can switch it on as needed.
Why use forms?
Your teams still fill in paper checklists or Excel sheets sent by email after each activity. Data arrives late, incomplete or hard to cross-check, and preparing a review or an audit means re-entering everything by hand.
What forms change in eBrigade
You create your forms from the administration area and link them to the relevant activity types. They then appear directly on the event page, with a completion progress bar. Each response is recorded with its author’s name and a timestamp, can be viewed in a consolidated view and exported to a multi-sheet Excel file with a statistical summary.
What you can do with Forms
Build your forms with short text, comment, yes / no, single choice, multiple choice, list, date and time questions, without code. Or import a template in JSON format, such as the vehicle inspection checklist provided: the editor fills in, you check, then you save.
Link each form to one or more activity types. It then appears directly on the event page, in a card showing the assigned forms and their completion progress bar.
Each response is recorded with its author’s name and a timestamp. When a form is reopened for an event, existing responses are pre-filled so they can be completed or corrected.
View all the responses collected for a form across every event, each with the respondent’s name and the time of entry.
Generate an Excel file with one sheet per question, plus a summary sheet presenting statistics and response percentages. The export can be filtered by period.
Seven separate permissions (view, create, edit and delete forms; view and export, edit and delete responses) govern use. An audit log records every operation and can be filtered by action type and activity type.
Step by step
From the administration area, add your questions from eight types: short text, comment, yes / no, single choice, multiple choice, list, date and time, or import a JSON template. Give the form a name.
Link the form to one or more activity types. It then appears automatically on the page of each relevant event, in a dedicated card with a progress bar.
Authorised members open the form from the event page and enter their responses. When it is reopened, responses already recorded are pre-filled so they can be completed or corrected.
See all the responses collected for a form, with the respondent’s name and the time of each entry. Export everything to a multi-sheet Excel file with a statistical summary sheet.
Where this module really pays off
Fire and rescue services (SDIS) & field teams: report or inspection forms linked to an activity type, filled in from the event page.
Training centres & apprentice training centres (CFA): end-of-session review linked to the training activity type, one response per session, with multiple choice and lists, exported to Excel for analysis.
Associations & volunteering: structured collection of lessons learned and activity reports, consolidated by form.
Quality & audit: multi-sheet Excel export with a summary sheet, response percentages and a filterable audit log.
Multi-activity coordination: one form linked to several activity types, tracked through the progress bar on each event.
Frequently asked questions
Who can create, edit or delete forms?
Permissions are granular: seven separate permissions govern viewing, creating, editing and deleting forms, viewing and exporting responses, editing them and deleting them. You assign these permissions according to the profiles in your organisation; on an activity they add up with those of the participation type, so a participant can answer without any general right on the module.
How is a form made available to members?
You link the form to one or more activity types. It then appears automatically on the page of each relevant event, in a dedicated card with a completion progress bar.
What field types can I use?
Eight types are available: short text, comment, yes / no, single choice, multiple choice, list, date and time. You can also start from a template imported in JSON format, such as the vehicle inspection checklist provided, then adapt it before saving.
Can responses be exported for an audit or a quality report?
Yes. The export generates an Excel file with one sheet per question and a summary sheet presenting statistics and response percentages, and can be filtered by period. Every operation is also recorded in an audit log.
What do you see when viewing responses?
A consolidated view brings together all the responses to a form across events, with the respondent’s name and the time of each entry. Responses can be cleared for a specific event or for all of them.
If a form that has already been filled in is reopened, does everything have to be re-entered?
No. When a form is reopened for a given event, responses already recorded are pre-filled. You complete or correct them without starting from scratch.
