Les CPTS

CPTS ( Territorial Health Professional Communities) are an organizational method that brings together primary and secondary health professionals in a territory. This federation gives them the opportunity to coordinate their professions and, thus, make care accessible to patients. The CPTS act in a given territory and there are currently 738 in France.

In concrete terms, what is a CPTS?

CPTS are territorial organisations that are at the initiative of health stakeholders. They were created in 2016 as part of the law on the modernization of the health system. The objective of a CPTS is to make care accessible to patients with different needs thanks to the plurality of health actors who are mobilized. In reality, the provision of CPTS in the different territories responds to inequalities and problems of access to care. Indeed, for some territories, medical care could be complicated and not very accessible. The CPTS then respond to this problem and to the needs expressed by citizens. The CPTS’ areas of action are defined by health professionals according to the needs they identify.

Indeed, this gathering of several health professionals aims to facilitate access to care, offer a health pathway, and this, in the same place. The treatment is facilitated for the patient who can benefit from several treatments. CPTS stand out by responding to the needs of a population.

For health professionals, being part of a CPTS is also of interest. Indeed, this coordination of their services makes it possible to mobilise their skills, support each other and accompany each other in a common project.

The CTPS can take advantage of the support of the URPS (regional union of health professionals) to contribute to the project. The URPS provide health professionals to participate in the missions of the CPTS.

Who is part of a CPTS

This care system is made up of health professionals from different health professions , but all of them are essential in order to improve the provision of care:

  • Liberal professionals
  • Health centres
  • Medico-social actors
  • Nursing homes
  • Hospital home services
  • Prevention actors
  • Care teams (primary and specialized)

What are the missions?

The CPTS have many missions entrusted to them and are the subject of challenges in their own right. These missions include:

  • Improving access to care

This mission is undoubtedly one of the primary motivations of health professionals, which is to make care accessible. Indeed, from one territory to another, access can be compromised for a good number of cares. So, the CPTS are mobilizing to remedy this difficulty. This mission responds in particular to the need to access a general practitioner or unscheduled (emergency) care.

  • Organisation of multi-professional pathways around the patient

Ensuring a follow-up and a pathway for each patient. Management must be carried out in accordance with the patient’s needs. The challenge is also to promote home care, especially for the most fragile patients.

  • Development of territorial prevention actions

Depending on the needs of the territory of action, set up prevention, screening and health promotion actions.

  • Severe Crisis Response

Define an appropriate action plan for health crises , whether epidemics, attacks, nuclear accidents, etc.

  • Quality and relevance of care

To promote the sharing and exchange of good practices in order to deliver quality medical and nursing services.

  • Support for health professionals in the territory

Facilitate the reception and installation of health professionals.

What are the advantages of a CPTS

The implementation of this multi-professional coordination mode has many advantages for all its stakeholders:

  • For healthcare professionals

For practitioners, this organization brings many positive points. First of all, they have the opportunity to coordinate their activity with other health professionals while supporting each other. Thus, they mobilize their expertise for the purpose of a common project. Also, they have access to better organization thanks to a follow-up of their patients that is faithful to reality.

  • For patients

Patients benefit from follow-up and consultations adapted to their needs. They benefit from optimal and rapid care and care. They overcome the problems of access to care.

  • For local authorities

The local authorities maintain an efficient medical activity in the territory. At the same time, they reduce inequalities in access to care while developing the skills of health professionals.

The financing of a CPTS

The funding that a CPTS can receive can come from different actors. Aid can be provided by the ARS (Regional Health Agency), by the CPAM (the primary health insurance fund) and finally by the local authorities. Some funding is granted for the creation of a CPTS and others for its development.

It should be noted that the amount of funding depends on the size of the population covered by the CPTS.
